Job Summary

We are seeking an experienced FHIR Architect to lead the design and implementation of CMS Interoperability and Patient Access Rule solutions. The ideal candidate will have deep expertise in HL7 FHIR standards, healthcare data exchange, and API-driven interoperability, ensuring compliance with CMS mandates while optimizing data accessibility, security, and performance.

Key Responsibilities

Architecture & Design

Develop the FHIR-based interoperability architecture in compliance with CMS Interoperability and Patient Access Rule.
Strong understanding of RCM and respective flow.
Knowledge on Payer to Payer exchange implementation and integration.
Should have understanding on Interop platforms and its inheritance.
Design and implement FHIR APIs for secure data exchange between payers, providers, and third-party applications.
Define FHIR resource mappings, profiles, extensions, and implementation guides.
Ensure alignment with HL7 FHIR, USCDI, Da Vinci, CARIN, and other relevant frameworks.
Technical Implementation

Lead the development of FHIR API Gateway, data repositories, and backend services.
Work with FHIR servers (e.g., HAPI FHIR, Firely, Microsoft Azure FHIR, AWS HealthLake, Google Healthcare API).
Implement OAuth 2.0, SMART on FHIR, and OpenID Connect for secure authentication and authorization.
Integrate FHIR-based solutions with EHRs, claims systems, and payer platforms.
Compliance & Standards

Ensure adherence to CMS, ONC, and HIPAA interoperability requirements.
Implement Blue Button 2.0, Da Vinci Payer Data Exchange (PDex), and Bulk FHIR API standards.
Establish best practices for FHIR data validation, security, and consent management.
Collaboration & Leadership

Work closely with health plans, providers, state agencies, and third-party developers.
Provide technical leadership to developers, data engineers, and integration teams.
Advocate for FHIR adoption and interoperability best practices within the organization.
Required Qualifications

Bachelor's or Master's degree in Computer Science, Health Informatics, or a related field.
7+ years of experience in healthcare IT with at least 3+ years in FHIR architecture and implementation.
Strong knowledge of FHIR R4, US Core Profiles, and HL7 standards.
Experience with FHIR servers (HAPI, Smile CDR, Azure API for FHIR, etc.).
Proficiency in RESTful API development, JSON, XML, and data transformation.
Familiarity with FHIR IGs (Implementation Guides) like CARIN, Da Vinci, US Core.
Expertise in security standards (OAuth 2.0, JWT, TLS, SMART on FHIR, RBAC/ABAC).
Experience with CMS interoperability mandates, payer-provider data exchange, and Fast Healthcare Interoperability Resources (FHIR) workflows.
Strong background in cloud-based healthcare solutions (AWS, Azure, Google Cloud Platform).
Understanding on FHIR data mapping and understanding of business logics.
Well versed the healthcare payer datamodels.
Preferred Qualifications

Hands-on experience with FHIR testing tools (Inferno, Touchstone, Postman, etc.).
Knowledge of claims data, EDI (X12 837, 270/271), and HL7 v2/v3 integration.
Experience with data streaming (Kafka, Apache Camel) and AI/ML applications in healthcare.
Certification in HL7 FHIR, AWS Healthcare Architect, or similar is a plus.
